ALTERNATE TRAVEL FORM

-AWAY RACES-

Under special circumstances, an athlete may be granted permission, by their Head Coach, to pursue alternate travel plans.

NOTE: Not all requests are automatically approved. The NAC Staff fosters team building and considers traveling as a team an important component of this team building process.

  • All alternate travel itinerary and trip details must be submitted to the Head Coach two weeks prior to departure date for final approval.
  • If approved, plans must involve travel with the athlete’s parents ONLY.  If parent is unavailable to travel, an alternate adult must be approved by the parent and the Head Coach.
  • Any additional travel arrangements or costs (i.e. shuttles, taxis, airfare) that are required as a result of the athlete’s alternate travel plans are the sole responsibility of the parent.
  • The NAC staff will not be involved or responsible for any alternate travel arrangements.
  • The athlete will not be considered under the supervision of the NAC staff upon removal from the event or upon delivery to the event, and the appropriate staff member has been notified and acknowledged.
  • Should a request for alternate travel be denied by the Head Coach, the athlete and parent may appeal to the Executive Director to render a final decision on the matter.

I understand and acknowledge that my junior athlete participating in the event shall be deemed to have waived all claims against the NAC, it’s officers and employees or parent volunteers for any injury, accident illness or death occurring during or by reason of the trip.
