ASH Productions Release & Waiver of Liability Agreement

I, represent, acknowledge and accept that participation in Artists Simply Human workshop includes strenuous physical activity and involves possible risks of serious injury (including paralysis and even death) with or without the negligence of others. Attendee, on his or her behalf, and the parent/guardians, on behalf of Minor as well as on their own behalf, unequivocally agree to incur and assume such risks as a condition to participation in the activities for which Attendee is registered.

 

I (Attendee or parent/legal guardian of Minor on behalf of the Minor and on their own behalf) hereby waive all claims (past, present or future), release and discharge, covenant not to sue, and agree to hold harmless ASH Productions, LLC (as well as its officers, employees and agents) and the hosting site on whose premises the event will be held (hereinafter collectively Releasees) from any and all liability, expense, loss, claims, action, demands, judgments and executions which the undersigned experienced or experiences in any way caused by, related to, or arising out of, directly or indirectly, the activity for which the Attendee is permitted to engage. In the event of illness or injury, I authorize ASH Productions, LLC to obtain necessary medical treatment for the Attendee and hereby, in my own behalf and on behalf of the Attendee, release and hold harmless all Releasees. This release covers every possible injury or accident of every sort and nature, whether related to the permitted activity, the equipment, the condition of the premises, or otherwise, and regardless of whether due in whole or in part to the negligence of a releasee or other Attendee. The undersigned represents and agrees that this waiver and release is binding not only on the undersigned, but also upon their respective heirs, representatives, administrators, executors, and assigns.

While the facility and event are both operated and maintained pursuant to applicable guidelines published by the Center for Disease Control, due to the nature of the COVID-19 virus no assurance or presentation is made that the facility or event and all employees, participants, guests, vendors, service providers and others that enter upon the facility and event are free from COVID-19. Accordingly, everyone entering upon the facility, event, or any part thereof assumes the risk of contracting COVID-19 and/or other viruses related hereto.

The undersigned represents that the Attendee is healthy and has no physical or mental condition that would impair Attendees ability to fully and safely participate in the intended activity.

All attendees under the age of 18 are required to have a chaperone age 21+ who will be responsible for the attendees at all times. ASH Productions, LLC is not responsible for attendees supervision.

I am aware that this is a release of liability and a contract between myself and ASH Productions, LLC, and sign it of my own free will. I have carefully read this agreement and fully understand its contents. If signed by Parent or Legal Guardian, I verify that the risks of the activities and the significance of this Release and Waiver were explained to the Attendee and that the Attendee understood them. I have also read the Refund/Credit policy in its entirety and agree to its contents.
