CHILDCARE LIABILITY WAIVER FORM
Childcare Policies & Procedures

This is a service we are offering, however it is NOT a daycare.  This is a safe place to let your children hang out while you attend a class. There are some important and non-negotiable RULES regarding the usage of the playroom: 

  • If a child needs to use the bathroom, parents will be notified and asked to accompany the child and return the child to the playroom before returning to class.
  • Any messes must be cleaned up before you and your child/children leave.  
  • We are not responsible any personal items that are brought with your children in to the playroom.  
  • You are responsible for your children, not us.  If your child is crying or needs attention stop your WOD and please take care of it.
  • The child cannot be disruptive to the training process or become a hazard on the floor.  If this is the case please rectify the issue immediately.  
  • Once your child is in the playroom they are required to remain in the room until you are ready to leave. 
  • NO FOOD/DRINKS except water in the playroom. 
  • NO DRAMA: Kids will be kids, but we expect the parents to ensure things do not get out of control and that they are showing respect to the playroom, the coaches and other children in the room.  If too many conflicts and grievances arise between children and/or parents we will ask that you do not bring your children with you. 
  • NO SICK KIDS: If they are too sick for daycare or school they are too for the playroom.  Runny noses, coughing, pink eye, fever, head lice, jungle rot – whatever they’ve caught we do not want to catch it too.  
  • NO KIDS ON THE GYM FLOOR:  This is not negotiable.  We cannot have kids distracting members while working out and we do not want your children to get hurt.  
  • NO KIDS ON THE EQUIPMENT:  No rings, no ropes, no rowers, nada.  

As the owners, we want to provide you with a great atmosphere to enjoy CrossFit and not have to worry about your children.  We are happy to provide this space for your children and we appreciate that you and you children will abide by the rules. 

Liability Waiver 

I, the undersigned, hereby hold harmless, waive and release The Pack CrossFit, Southern Idaho Brazilian Jiu Jitsu, their coaches, employees, volunteers, officers, representatives, agents, organizers, and successors from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with the participation of the kids room, including those allegedly attributed to the negligent acts or omissions of the above mentioned parties. I understand that the childcare services are provided only while I am present in the building and taking class. I understand that if my child becomes inconsolable during the class session, I am responsible to leave class and attend to my child. I understand that children are not allowed in the gym workout area at anytime. 

I have read and understood the foregoing assumption of risk, and release of liability and I understand that by signing it obligates me to indemnify the parties named for any liability for injury or death of any person and damage to property caused by the above mentioned parties negligent or intentional act or omission. I understand that by signing this form I am waiving valuable legal rights.
