GYM LIABILITY WAIVER

PLEASE READ CAREFULLY! THIS IS A LEGAL DOCUMENT WHICH AFFECTS YOUR LEGAL RIGHTS

WAIVER AND RELEASE OF LIABILITY You certify that you have no current medical, physical, or mental disabilities that would prevent you from performing physical exercise, and/or being able to follow our nutritional program, acknowledge that you are voluntarily participating in this program and will adhere to our guidance. Please obtain a physical examination from your physician before using exercise equipment or participating in our nutritional program. You understand our program can be modified by your physician if necessary. You are responsible for complying with any restrictions involving exercise or diet that is ordered by your physician. You agree to refrain from the consumption of alcohol or mind altering drugs prior to participation of any exercise. Your participation in this program shall be at YOUR OWN RISK! You agree to accept full responsibility for your participation, and forever release the Club from any lawsuits, claims, demands, injuries, damages orations arising due to injury to you or your property arising out of, or about the services, facilities, and premises of EveryBody Fitness.

You hereby hold EveryBody Fitness, its officers, owners, agents, franchisees, suppliers and employees harmless from any and all result from any injury, illness, or death, accidentally during, or arising in any way from your participation at any Club location (be it accept full responsibility for your participation, and forever release the Club from any lawsuits, claims, demands, injuries, damages orations arising due to injury to you or your property arising out of, or in connection with the services, facilities, and premises of EveryBody Fitness, claims that may corporate or franchise location). You acknowledge you have read and received the attached Notice of Cancellation, and Rules and Regulations (available upon request), which are part of this contract, AND have been informed orally of your cancellation rights.
