Climbing Wall

* I understand that a number of activities and events offered by the Sports Club, including this organized program, may involve inherent danger and risk of personal injury. I accept these risks on behalf of myself and my child(ren).

I Agree

*In consideration of the benefits to be derived by me and my minor children (if any) from the use of the recreational and exercise equipment and facilities provided by the Sports Club, I agree, on behalf of myself, my children under the age of 18 years, my heirs, executors, administrators, successors and assigns, that the Sports Club shall not be liable for any damages arising from personal injuries (including death) sustained by me or my minor children in, on, or about the Sports Club's premises, or as a result of the use of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from negligence of the Sports Club.

*I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type that may occur to me or my minor children, and I fully and forever release amd discharge the Sports Club from any and all claims, demands, damages or causes of action, resulting from or arising out the use of such equipment and facilities.

*I agree to indemnify and hold the Sports Club harmless from and against any and claims, demands, damages, rights of action, causes of action, or expenses (including attorney's fees) of any person or entity, that may arise from injuries or damages sustained by me, my minor children or other family members or my guest.

I understand that the Sports Club does not provide supervision, instruction or assistance for the use of all the facilities and equipment at all times and I accept these risk on behalf of myself and my child.

Sunscreen Application Authorization 

We must have permission to apply sunscreen to your child. You will need to supply the sunscreen, clearly labeled with your child's name.

Swimming/Wading Permission 

I give my son /daughter, permission to participate in the swimming and wading activities provided by the Boar's Head Kid's Club. I understand that my child will wear a life vest provided by the club for safety if he/she is 4 or 5 years old or under regardless of his/her swimming skills. I give my child(6 or older) permission to swim in the round pool and racing pool. I acknowledge that my child will wear a life vest unless otherwise stated below

Movie Permission 

The children stay very busy through the summer with sports and outdoor activities. To give the children the opportunity to relax and cool down, movies will be offered as an option for a special treat or rainy days in the afternoon. Parents may elect to sign the permission slip for their child or recommend their involvement in the other activities offered at that time.

Social Media and Publicity Release 

I consent to the use of my photograph, for advertising and publicity purposes relating to the Boar's Head Inn. This consent extends to the following media, without further notice, permission, or compensation, and without territorial or time restrictions.

I am the parent/legal guardian of "Minor" I have read the Participant Agreement, Release, and Acknowledgement of Risk in its entirety and I understand the potential risk in indoor rock climbing. I grant permission for my child to participate in this activity. I certify that I am legally competent to grant permission as an adult and warrant my authority as the parent/legal guardian.
